Where things stand, in three lines

What happened on 11 August 2026
At an awards ceremony in Salt Lake City, the XPRIZE Foundation announced the 20 Finalist teams in XPRIZE Healthspan. At the same event, 10 of those teams were named Milestone 2 Awardees and received $1 million each, totalling $10 million.
Alongside the money, awardees gain access to clinical testing resources. That matters more than it sounds: the bottleneck in ageing research has rarely been ideas — it has been the ability to measure and compare them rigorously.
The finalists span the United States, Japan, South Korea and China.
What XPRIZE Healthspan actually is
The XPRIZE Foundation is a US non-profit that uses prize money to accelerate technology — best known for private spaceflight (Ansari XPRIZE) and carbon removal.
XPRIZE Healthspan launched in 2023 as a seven-year competition with a total purse of $101 million. Co-title sponsors are Hevolution Foundation (Saudi Arabia) and SOLVE FSHD, with GSK as pharmaceutical industry sponsor.
How the prize is structured
This is the part most often misreported. The grand prize is not split into first, second and third place. The amount depends on how many years of function a team restores.

What gets measured
Not a single biomarker. Teams must show improvement across three functional domains:
- Muscle function
- Cognitive function
- Immune function
And the constraint is severe. Treatment must take one year or less, and that single year of intervention must turn the clock back by at least 10 years, with a goal of 20. “Take it for decades and it might help” does not qualify.
Participant age is given as 50–80 on the official competition page; the August press release describes “adults aged 50 to 90”, so the published figures vary slightly.

What the 20 teams are actually testing — a digest by approach
This is the heart of it. “Slowing ageing” sounds like one thing, but these teams are aiming at completely different targets. Here they are grouped into seven families: what they use, where in the body it acts, and what result is expected.

1. Stacked existing drugs
AgelessRx (USA) — a telehealth protocol stacking rapamycin, metformin, low-dose naltrexone, NAD+, glutathione, sermorelin and tirzepatide alongside resistance training and health coaching. The targets are mTOR inhibition, AMPK activation and NAD+ repletion.
NYC-Vita (USA, Mount Sinai Health System) — low-dose rapamycin plus spermidine plus structured HIIT and resistance exercise. Rapamycin inhibits mTOR; spermidine restores autophagy, the cell’s self-cleaning process. Mount Sinai frames its primary aim as rebalancing immune function.
Expected benefit: better strength and body composition, a rejuvenated immune profile. Strength: every drug already has human safety history. Weakness: with many agents at once, attributing any effect to a specific one is very hard.
2. Extracellular vesicles — where both Japanese awardees are playing
Cells release tiny capsules (exosomes among them) loaded with miRNA and proteins to signal other cells. Both Japanese Milestone 2 teams want to turn that into therapy.
Goda Lab (Japan — University of Tokyo, Tohoku University, NanoTitan, Tokyo Relife Clinic) — engineered extracellular vesicles derived from stem cells, using what the team calls “super homotypic targeting” to deliver regenerative miRNAs and proteins specifically to muscle, neural and immune tissue. Notably, this aims at all three scoring domains with a single modality — a good fit for the competition’s rules.
TIME TRAVELER – Plant-EVs (Japan) — parsley-derived plant extracellular vesicles taken as a dietary supplement, selected for anti-inflammatory and anti-senescence properties. Not an injection or a prescription: it is delivered as something you eat. If it works, the barrier to adoption is close to zero.
What is interesting: same biology, opposite strategies — Goda Lab is building precision medicine, TIME TRAVELER is building a food product.
3. Stem cells
Longeveron (USA, NASDAQ listed) — laromestrocel (Lomecel-B), allogeneic mesenchymal stem cells from the bone marrow of young healthy donors, with pro-vascular, immunomodulatory, tissue-repair and neuroinflammation-suppressing actions.
Already demonstrated: published trial results showing increased 6-minute walk distance in patients with aging-related frailty — which is what earned the Milestone 2 award. Among the 20, this is one of the most concrete sets of human outcome data.
4. Gene therapy
Minicircle (USA) — non-viral plasmid gene therapy given by subcutaneous injection, expressing follistatin and klotho. Follistatin blocks myostatin, the protein that restrains muscle growth; klotho acts through the FGF23 pathway and is linked to kidney function and ageing.
Expected benefit: increased muscle mass and strength. Caveat: the company has previously offered unapproved treatment in lightly regulated jurisdictions, so this trial is the first rigorous test of both efficacy and safety.
5. Antibody drugs
Johns Hopkins-Suninflam (USA) — SIF001, a monoclonal antibody against galectin-3, a protein involved in chronic inflammation and fibrosis. Blocking it is intended to suppress chronic inflammation and amyloid-beta aggregation in Alzheimer’s disease.
Expected benefit: preserved or improved cognition. This is the most conventional drug-development route in the field — and correspondingly the slowest and most expensive.
6. Fixing cellular machinery
Mitochondrial All Stars (USA — Mighty Therapeutics, University of Washington) — elamipretide (the SS-31 peptide), which binds cardiolipin in the inner mitochondrial membrane to stabilise it and restore energy production. The premise: declining mitochondrial function underlies age-related fatigue and weakness.
Progerinin (South Korea — PRG S&Tech, team RPRGAON-Progeria) — a small molecule that blocks progerin from binding lamin A, driving progerin degradation and restoring the architecture of the nuclear lamina. Originally developed for progeria, but progerin also accumulates in ordinary ageing, which is the basis for the wider application.
7. Diet, lifestyle and traditional materials
RETRO-EPIGERNA (China, Macau University of Science and Technology) — the KYN Composite Capsule, combining Chinese herbal medicine with probiotics, targeting tRNA epitranscriptional modification to improve proteostasis and mitochondrial function. An attempt to restate traditional materials in modern molecular terms.
Japan Longevity Consortium (Japan — Juntendo University, University of Tokyo, Osaka University) — led by Professor Shigeo Horie of Juntendo University. Combines time-restricted meals built on traditional Japanese cuisine, lifestyle design for autonomic balance, gut microbiome management and nutritional intervention, and even social connection, evaluated using biological-age biomarkers. The team includes not only clinicians and researchers but gym operators and culinary experts.
Expected benefit: not one dramatic effect but many small gains stacked together. This family faces the lowest regulatory barrier — if it wins, it reaches the public fastest.
The remaining finalists
XPRIZE’s own press release does not itemise the teams beyond the ten Milestone 2 awardees. Industry outlet longevity.technology reports that the twenty include Morinaga Healthspan (Japan), Japan Longevity Consortium (Japan), Abe Yoando Pharma (Japan), NUS Academy for Healthy Longevity (Singapore), GI Innovation (South Korea), ANI Biome, Boston Healthspan Team, ASU Team Healthspan and GOQii Sanjeevini (India). We have not confirmed these names against a primary source; treat the exact wording as provisional.
Of these, Abe Yoando Pharma states on its own site that its intervention uses NMN (nicotinamide mononucleotide), that it reached the Top 40 in May 2025, and that it submitted clinical data in June 2026.
For the trials themselves, the University of California San Diego has reportedly been named as the testing hub — a central evaluation base rather than each team measuring in its own way, which should make results genuinely comparable.
What the whole field tells you
- Muscle is the most crowded target — of the three scoring domains it is the easiest to quantify (grip strength, gait speed, 6-minute walk distance)
- Few teams attack cognition head-on — reversing a decade of cognitive ageing in one year is among the hardest things in medicine today
- The Japanese entries cluster around things you swallow — vesicles, NMN, Japanese-cuisine-based meal design. Rather than meeting US capital head-on in injectables and gene therapy, they compete on food and fermentation
- Very few have published human outcome data yet — the most concrete so far is Longeveron’s 6-minute walk distance
Japan’s showing
Of the 10 teams awarded $1 million, two are from Japan.
Japan’s participation was substantial from the start. In a June 2025 interview, Dr Hidekazu Yamada, chairman of the Japanese Society of Anti-Aging Medicine, said that 25 Japanese entries competed (19 in the Healthspan track, 6 in FSHD), and 8 reached the Top 40 — six in Healthspan and two in FSHD. Some teams announced their own results publicly; Tokai University, for instance, published its Top 40 selection.
Goda Lab (University of Tokyo, Tohoku University and partners)
An academia–industry consortium combining university research with NanoTitan and Tokyo Relife Clinic. Building a structure that can carry laboratory findings all the way into human testing is very likely why this team survived to this stage.
TIME TRAVELER – Plant-EVs
“Plant-EVs” refers to plant-derived extracellular vesicles — tiny membrane packages that cells release to exchange signals. A research field has grown up around the question of whether vesicles from plants we eat can act on human cells.
What distinguishes the Japanese entries as a group is their food-and-fermentation angle. According to the teams’ own announcements, traditional fermented foods and teas — Awa bancha (a lactic-acid fermented tea), matcha, natto and amazake — are among the candidates under evaluation for slowing ageing.
In a field dominated by American capital, it is notable that Japan has reached the final stage on a distinctly different premise. But this does not mean Japanese foods have been shown to stop ageing. The trials run to 2029, and judgement comes after that.
The real test: clinical trials, 2026–2029
Being named a finalist is an assessment of promise. The actual contest starts now. Each team will run trials of up to one year through 2029.
What makes this design unusual is that very different approaches will be compared against one identical yardstick. Cellular reprogramming, stem cells, repurposed drugs, plant-derived vesicles — claims that have until now been made on incompatible endpoints will be measured head to head, arguably for the first time.
That is why the 2030 result will matter as much for what fails as for who wins. Ageing research has produced a long line of promising hypotheses that quietly disappeared, largely because fair, large-scale comparisons were never run.
⚠️ What this means for you right now
Plainly: there is nothing here that should change what you do today.
- Every finalist therapy is in clinical trials. Neither efficacy nor safety is established
- Results arrive in 2030, and regulatory approval would add years beyond that
- Being “an XPRIZE finalist” is a judgement of promise, not proof of effect
One thing to watch out for
This news is easy to exploit commercially. Expect supplements and private clinics to start advertising “technology from an XPRIZE finalist” or “an ingredient recognised in a $100 million competition”. Some finalists have offered unapproved treatments in jurisdictions with light regulation — entering a competition and being worth paying for today are entirely separate questions.
A simple rule: if “we’re in XPRIZE” is the selling point, that is marketing, not evidence. Ask instead what the team has published in peer-reviewed journals.
What actually works, today
You do not have to wait for 2030. Interventions with established, large-scale evidence already exist — and unglamorously, none of them required a $100 million prize.
- Stopping smoking (the largest single-intervention effect size there is)
- 150–300 minutes of aerobic exercise per week, plus resistance training 2–3×
- Keeping blood pressure under 120/80 mmHg
- Adequate sleep, and consistent sleep–wake times
- A Mediterranean dietary pattern; limiting ultra-processed foods
- Maintaining social connection

- 2026–2029: whether teams publish interim results in peer-reviewed journals — a useful signal of seriousness
- Why teams drop out: if further teams fall away, the reason (efficacy, safety or funding) will itself be valuable information for the field
- Japan’s entries: whether food- and vesicle-based approaches hold up under a standardised comparison
- 2030: the grand prize announcement. “No winner” remains a realistic outcome — restoring a decade of function is that hard
